2027 CalPERS medical changes

What is changing

  • UnitedHealthcare Basic HMOs are ending. SignatureValue Alliance and SignatureValue Harmony will no longer be CalPERS Basic plan options effective January 1, 2027.
  • Family-building benefits are expanding. Beginning July 1, 2027, all CalPERS Basic HMO and PPO plans will include infertility diagnosis and treatment coverage, including applicable IVF benefits.
  • Sutter Health Plan is being added in select counties. The new Basic HMO will be available in Placer, Sacramento, San Joaquin, Stanislaus, Solano, and Yolo counties. Plan availability varies by location.

Premiums and choosing a plan

5.78% CalPERS weighted-average Basic-plan premium increase for 2027.

That is only a program-wide average. Your actual cost depends on the plan you choose and the District contribution, so compare your options directly.

When using the CalPERS health-plan search: select Public Agency & School Members, then enter your ZIP code.

Why am I required to log into AFenroll if I'm not making changes?

Open Enrollment is a good time to review and confirm your information and benefits. Employees experience life changes but neglect to notify Human Resources or fail to update benefits.

Here are things you should review that impact your benefits:

  • Address
  • Marital Status
  • Covered dependents (is a dependent missing or should one have been removed?)
  • Dependent information (is the date of birth correct?)
  • Re-Enroll in a Flexible Spending Account (FSA)
I need help navigating the benefits platform.
What changes can I make?
  • Add/Delete eligible dependent(s) from your plan (medical, dental and vision).
  • Cancel coverage.
  • Change health plans (medical or dental).
  • Enroll/Re-Enroll in a Flexible Spending Account (FSA) plan.
  • Enroll in Voluntary Benefits.
I don't have any changes for 2027. Do I need to do anything?

YES. All benefit eligible employees will need to log into the Online Benefits Platform (AFenroll) to review and confirm their 2027 benefit elections.

Do I have to meet with American Fidelity?

No. You can log into the online benefits platform to review, make changes, and confirm your benefits on your own. See the AFenroll Self-Service Instructions  .

I would like to make a change to my benefits. What forms do I need to complete?

All changes will need to be submitted through the AFenroll online benefits platform. Whether you are adding/deleting a dependent, cancelling coverage, enrolling, or changing plans, submit your change through the online benefits platform. No paper forms will be accepted.

What health plans are available in 2027?

Available CalPERS medical plans vary by location. Use the CalPERS Health Plan Search by ZIP CODE to see the plans available in your area.

On the CalPERS site, select Public Agency & School Members, then enter your ZIP code.

How do I choose a plan?
  1. Use the CalPERS Health Plan Search by ZIP CODE. Select Public Agency & School Members, then enter your ZIP code.
  2. Review the 2027 District Medical Contribution Rates  . Rates may be updated before January 1, 2027, so use the most recently posted rate sheet.
  3. Review the 2027 SMCCCD Benefits Guide   to compare health plans, costs, services, doctors, and more.
  4. Check with your doctor's office to confirm which plans your doctors are preferred providers (in-network).
I'm adding dependent(s). What documentation do I need to provide?

If you are adding dependent(s) to your plan(s), the following documentation is required:

  • Spouse – marriage certificate
  • Domestic Partner – Domestic Partnership registered with the state and affidavit
  • Dependent child(ren) – birth certificate

Submit the applicable documentation via secure Dropbox.

How about FSA? How do I make my election for 2027?

Enrollment for Healthcare and Dependent Care Flexible Spending Accounts (FSA) can be done through the online benefits platform.

I would like to cancel coverage or remove a dependent. What do I do?
